Tax File Number Notification
If you are buying, selling or transferring securities in entities managed by Peet Limited, you will also need to provide your Tax File Number (TFN) using the TFN Notification Form, if you have not done so already. If we don't have your TFN, we may be required by the ATO to withhold tax on future payments at the highest marginal rate.

Change of Details
If any of your contact details have changed, please sign and complete the Change of Details form and return it to our office. If the amendment is a change of name, please send a certified copy of any document that would support this change with the form, e.g. marriage certificate, etc.

Direct Credit Details
If you wish to receive payments direct into your bank account, please complete the Direct Credit form and return it to our office at your earliest convenience.

Replacement Certificates
If you cannot find your original share/unit certificate, a replacement certificate can be reissued at your request. Please complete the Lost or Destroyed Share Certificate statement and return this by post. Upon receipt of this statement, the necessary changes will be made to the register and the reissued certificate will be sent to you. Please note if you require a replacement certificate for any of the Computershare managed entities listed above please forward the Computershare form direct to them or contact the Investor Centre.

Buy, Sell or Transfer Securities (Syndicates/Funds)
If you wish to register your interest in buying or selling securities in any of the entities managed by Peet Limited, you will need to complete a Buyers' or Sellers' Authority form, for syndicates/funds as appropriate, and return it to us.
If you wish to transfer securities in entities managed by Peet Limited you will need to complete a Transfer Form and forward it to Peet Limited. Please ensure you return the original share certificate (or a Lost or Destroyed Share Certificate form if this has been misplaced) along with the transfer form and any certified documents where applicable. Please note that if you are transferring securities in any of the Computershare managed entities listed above then please complete the Computershare transfer form and return it direct to them.
